In order to complete this install please fill out the details requested below.
Please note the following points before you continue:
    1. Change the permission (chmod 777 for linux server) of 'includes/config.php'. (After installation dont forget to change it back to 644)
    2. Change the permission (chmod 777 for linux server) of 'images' folder. (After installation dont forget to change it back to 755)
    3. Change the permission (chmod 777 for linux server) of 'pie.png'.
    4. The database you install into should already exist.
    5. Provide a table prefix to distinguish the EasyWebSurvey tables from other tables or if you have some similar tables.
    6. After the install process delete the 'install' folder and its contents.
